At this open house hosted by the Faith & Health Board, trained Advance Care Planning facilitators will be available to answer your questions and assist you in starting, completing, revising, and/or notarizing your health care directive. A health care directive is a document for putting your health care   wishes into writing in the event you are incapacitated in the future. This is a way to make known your values and goals for care, and to gain peace of mind (and a thumbs-up from family members and health care workers).

Adults of all ages are welcome to come anytime during the 2-hour open house to meet with a facilitator. Feel free to bring your own blank or partially-completed health care directive; blank documents will also be available. This service is being provided free of charge by the Northfield Area End of Life Choices Matter Group.
